Surrey Immunisation Service is coming in to Wallace Fields Junior School to administer the flu vaccinations on 10th November 2026.   

Please can all parents complete the e-consent form by visiting www.surreyimmunisations.co.uk and enter your school’s unique code SR125298

The deadline for submitting a flu vaccination consent form is 4 working days before their visit.

If you do not want your child to receive a vaccine, please also complete the consent form and select the ‘No’ option.
